Your First Visit

Please bring your referral form from your general dentist to your appointment as well as the names of any prescriptions you may be taking and relevant health and dental histories.

If you have dental insurance, please provide this information to our office staff before your first appointment.  

You will be given the opportunity to fill out the initial forms/paperwork from the comfort of your home. This will help save time during your office visit.  If you do not have access to do this from home or feel more comfortable doing this at the office, no problem!  Please try to come 15 minutes earlier than your appointment time to complete these forms in our waiting room.
